Appointments are made only by phone. Calling lets the staff understand what the visit is for, reserve the appropriate amount of time, and explain what to bring.

Confirm insurance before the visit

Have the full plan name and member ID ready. Carrier names alone do not confirm network participation.

Insurance information

Bring current information

Bring photo identification, every current insurance card, a list of medicines, and any records or forms the office asks you to provide. Arriving a few minutes early gives you time to complete any paperwork before your visit.
