The exact plan matters more than the carrier name.

Insurance companies offer many products and networks. Please verify coverage before your appointment rather than relying on the name printed at the top of the card.

Participation must be checked for your exact product and clinician.

The information below describes plan categories the office commonly works with or is asked to verify. It is not a guarantee of payment or network participation.

Medicare

The office works with Original Medicare and is asked to verify a range of Medicare Advantage products. Medicare Advantage plans use separate commercial networks, so each product must be checked.

Patients also ask us to check Medicare Advantage products associated with Premera, Regence, Humana, Aetna, and UnitedHealthcare.

Washington Apple Health

Apple Health managed-care coverage must be verified by the plan shown on the member’s health-plan card. ProviderOne is Washington’s eligibility and billing system; it is not the name of a managed-care network.

Patients commonly ask us to check products from Molina Healthcare of Washington, Coordinated Care, and Wellpoint Washington. Bring both your ProviderOne card and managed-care plan card when you have them.

Commercial & employer coverage

Patients commonly ask us to check products associated with Premera, Regence, First Choice Health, UnitedHealthcare or UMR, Aetna, Cigna, LifeWise, Ambetter, and certain Kaiser PPO or Options networks.

A carrier name on this page does not mean every product from that carrier is in network.

Community & closed-network plans

Call before scheduling if the card says Kaiser Core or HMO, UnitedHealthcare Community Plan, or Community Health Plan of Washington. The office may not participate in these products.

How to verify coverage

  1. Call the office with the full plan name and member ID.
  2. Ask the insurer whether both Kutsy's Medical Practice and the clinician you will see are in network.
  3. Ask whether your visit requires a referral, authorization, deductible, or copay.
  4. Bring every current insurance card to the appointment.
Coverage is not the same as payment.

Benefits, deductibles, copays, authorizations, and claim decisions are controlled by the insurer and the terms of your plan. Credit cards are accepted for patient payments.
