Before the appointment
Call the office before your visit. The staff will explain which documents and the current edition of Form I-693 you should bring for your situation.

Bring relevant medical records and a current list of medicines.

The process
Requirements depend on age, medical history, vaccination records, and current federal instructions.

Dr. Kutsy reviews identification, medical history, vaccination documentation, and the examination requirements that apply.
Current USCIS and CDC instructions may require vaccination, laboratory screening, or another follow-up step. The office will explain what applies.
When the process is complete, Form I-693 is returned in a sealed envelope for USCIS. Do not open the sealed envelope.
Performs the medical examination, reviews required vaccination evidence, records the findings, and completes the medical portions of Form I-693 according to current federal instructions.
