A few things that help visits run smoothly.

These policies explain how scheduling and payment work at the office, and where to find your privacy rights.

Appointment scheduling

Appointments are made by phone so the staff can reserve the right amount of time and confirm what to bring. If you need to cancel or reschedule, we appreciate at least 24 hours' notice whenever possible, so that appointment time can be offered to another patient.

Financial policy

  1. Payment is due at the time of service.
  2. Credit cards are accepted for patient payments.
  3. Please verify your insurance coverage before your visit—see insurance information. Participation depends on the exact plan and network shown on your card.
  4. Patients are responsible for any charges not covered by insurance, including deductibles, copays, and services the plan does not cover.
